The provisional patent cover sheet functions as a table of contents for the application package. The provisional application cover sheet is available on the USPTO website forms page. You should select the SB16 EFSWEB form, which is much easier to handle electronically, rather than the standard SB16 form. If you choose the standard form, you will have to fill it out by hand, then scan and convert it to Adobe PDF format. Specific instructions on filling out the SB16 EFS-WEB form are also available on this page. You can type and sign the SB16 EFS-WEB form using the latest version of Adobe Reader, available for free at https://get.adobe.com/reader/.
Fee Transmittal Form
The fee transmittal is a form that is used to calculate and identify fees accompanying the provisional patent application. Only the basic filing fee is due for filing a provisional application. The basic filing fee depends upon the size of the entity filing for the provisional patent application.
Recordation Form Cover Sheet
If you wish to record an assignment of your provisional patent application electronically, you must use an entirely different system on the USPTO website, the Electronic Patent Assignment System (EPAS), available at http://epas.uspto.gov. You can do this whether you filed your patent application electronically or by mail, but you must have the patent application number on hand, which you will receive several weeks after you file a paper application, but momentarily when you file electronically.
